Is an injury default a loss?

Technically I believe so. One example of this happening I can think of was Jason Detre from LaRue his senior year. Blew out his elbow first match if the year and had to default. Came back at State Duels and ran off 18 straight wins en-route to his 2nd State Title. His official record that year was 18-1.
Yes. If you started the match and get hurt then it is a loss.
If you were hurt in the previous match and default the next without starting the match then it is not a loss.

What if you were in the semi-finals and won, but during the match an injury occurred. A sprained ankle for example and could not wrestle the finals. Does your opponent receive a head to head victory over you if you have to forfeit the finals match?

No, you did not step on the mat.
According to NFHS, if you wrestled in the bracket at any time, medical forfeits and injury defaults are counted as a loss. Not at collegiate level though. Unless something changed in last couple of years, it is an L on record.

it's a Medical Forfeit and it is a loss in an individual tournament. This happened to us a few years ago at the UHA tourney. Kid got hurt in the semifinals we won, had to MFF the finals. it is a loss.
